Living Proof HIV/AIDS Primary Care Program

Both J-CAP clients as well as local community residents who seek out our services receive preventive and educational information about HIV/AIDS and are tested upon consent. If a client is or found to be HIV positive, medical, psychological and case management services are available through J-CAP’s Living Proof HIV/AIDS Primary Care Program

At Living Proof, both the medical and emotional needs of those infected and affected by HIV/AIDS are addressed by a team of experienced medical and nursing staff.  A full range of services is offered including:                         
  • HIV Testing/Pre-and-Post Test Counseling for ages 13 and up 
  • Medical Exams
  • Health Monitoring and Treatment Adherence
  • Partner Notification
  • Support Groups
  • Referral Services
  • Priority Screening/Admission to Residential and Outpatient Chemical Dependency Treatment Programs
  • Nutrition Education
  • Health Education and HIV Prevention Seminars
  • Mental Health Services
  • Case Management
  • Women's Issues Groups
  • MSM (Men Having Sex With Men) HIV Negative Groups
  • Peer Educators/Buddy System
  • Community Outreach
